How to Upload a Chart

Log in and navigate to the Chart Reviews dashboard.

Click the "Upload File" button.

Select the chart file from your device.

Once uploaded, your collaborating physician will automatically be notified.

The dashboard provides real-time status and details about each chart uploaded, including:

Attestation Period

Date Reviewed
